What is Buprenorphine?

According to the National Alliance of Advocates for Buprenophine Treatment:

“Buprenorphine (BYOO-pre-NOR-feen) (‘bu-pre-‘nor-feen) is an opioid medication used to treat opioid addiction in the privacy of a physician’s office.1 Buprenorphine can be dispensed for take home use, by prescription.1 This in addition to buprenorphine’s pharmacological and safety profile makes it an attractive treatment for patients addicted to opioids.2

Buprenorphine is different from other opioids in that it is a partial opioid agonist3. This property of buprenorphine may allow for;

  • less euphoria and physical dependence*3
  • lower potential for misuse*3
  • a ceiling on opioid effects*3
  • relatively mild withdrawal profile*3

At the appropriate dose buprenorphine treatment may:

  • Suppress symptoms of opioid withdrawal2
  • Decrease cravings for opioids2
  • Reduce illicit opioid use2
  • Block the effects of other opioids2
  • Help patients stay in treatment2

* When compared with full opioid agonists (such as oxycodone and heroin)3

Buprenorphine (‘bu-pre-‘nôr-feen) (C29H41NO4) is a semi-synthetic opioid derived from thebaine, an alkaloid of the poppy Papaver somniferum. Buprenorphine is an opioid partial agonist. This means that, although Buprenorphine is an opioid, and thus can produce typical opioid effects and side effects such as euphoria and respiratory depression, its maximal effects are less than those of full agonists like heroin and methadone. At low doses Buprenorphine produces sufficient agonist effect to enable opioid-addicted individuals to discontinue the misuse of opioids without experiencing withdrawal symptoms. The agonist effects of Buprenorphine increase linearly with increasing doses of the drug until it reaches a plateau and no longer continues to increase with further increases in dosage. This is called the “ceiling effect.” Thus, Buprenorphine carries a lower risk of abuse, addiction, and side effects compared to full opioid agonists. In fact, Buprenorphine can actually block the effects of full opioid agonists and can precipitate withdrawal symptoms if administered to an opioid-addicted individual while a full agonist is in the bloodstream. This is the result of the high affinity Buprenorphine has to the opioid receptors. The affinity refers to the strength of attraction and likelihood of a substance to bind with the opioid receptors. Buprenorphine has a higher affinity than other opioids and as such will compete for the receptor and win. It will “knock off” other opioids and occupy that receptor blocking other opioids from attaching to it. If there is enough Buprenorphine to knock the opioids off the receptors but not enough to occupy and satisfy the receptors, withdrawal symptoms can occur; in which case the treatment is more Buprenorphine until withdrawal symptoms disappear.

In October 2002, the Food and Drug Administration (FDA) approved Subutex® (buprenorphine hydrochloride) and Suboxone® tablets (buprenorphine hydrochloride and naloxone hydrochloride) for the treatment of opiate dependence(addiction).  On October 9, 2009 the FDA approved a generic version of Subutex. In 2011 Subutex (brand name) was discontinued, in 2012 Suboxone (brand name) tablets were discontinued and replaced with Suboxone Film. In February 25, 2013 the FDA approved Suboxone generics. On July 6, 2013 Zubsolv (BupNx) sublingual tablets were FDA approved and became available in September 2013. In November 2014 Bunavail (bup/nx) buccal film became the next brand name bup/nx product to hit the market. In May 2016, Probuphine®, a buprenorphine under-the skin implant was FDA approved. It is installed subcutaneously and provides a steady dose of buprenorphine for 6 months.

Suboxone, Zubslov, and Bunavail contains both buprenorphine and the opiate antagonist naloxone. Naloxone has been added to guard against intravenous abuse of buprenorphine by individuals physically dependent on other opiates. If misused by injection, the naloxone (along with the buprenorphine itself) will help cause immediate withdrawal in physically dependent people, however when taken as directed, the naloxone is not well absorbed and is considered clinically insignificant.”

“Today was a great day, I have been dependent on OxyContin and Percocet for around 20yrs, sick of the roller coaster ride of addiction, not even remembering having a day that was not controlled by drugs, almost losing my life 3 times last year without any hope of a future, knowing that Drugs were not only controlling my life, but having a very negative effect on my family’s, Not only have I been a burden to my family, I also knew that I was very near death, but even though I wanted to get well and be the woman I once was, I really don’t remember how it even feels to be her anymore and how to stop, I was scared because from abruptly just stopping in the past, it put me into seizures, and I stopped breathing! Scared to death, I remembered talking to this woman after calling Dr. Readers office looking for help about a yr ago, and worried that I the woman that has raised to beautiful kids and had my own Childcare, soccer mom, Girl Scouts, you name it, was going to be judged, I thought I would be looked at like a failure. Well today I learned that I was wrong, Barbra’s encouraging words made me realize I am only human like everyone else, and that I could have enough courage to make my first step into the office that felt like I was welcomed in by old friends, and all my fear was gone, these girls were genuine and kind, you could tell that they really care, and not just because their paid to, and I’m sure most of us know from seeing so many different doctors to get our fix through the years how those places make you feel. I then met the most amazing doctor, he made me feel like I could tell him anything, and I did, I told him about all the years of drug abuse and how I really didn’t remember much of the last 2 yrs! Looking up at him as he listened patiently, I still just seen his warm and caring face, I mean his heart is truly about helping all of us that feel like we are not even worth helping! I know he has my best interest in mind, and I am trusting him with my life! Today for me was my last chance at living. The medicine he prescribed is already working,& I know God sent him and his staff to save me from myself! Thank You So Much! For anyone who is out there reading this, please just take the first step and call, when we are down as low as we can go , then we’ve got no where else to go accept up, right? So why not let the best of the best help you by taking your hand? WE CANNOT CHANGE WHAT WE DONT ACKNOWLEDGE! COME ON WE CAN ALL DO THIS TOGETHER, BECAUSE WITH GOD AND THOSE 3 ANGELS WE WILL GET THROUGH THIS! XOXO”

“I have been a patient of Dr. Reader for more than 2 years. I can still remember my very first office visit – meeting Barbara, Helen, and the Dr. What an amazing feeling! I can truly say that I have NEVER before been treated with such unconditional compassion, and dignity. The staff in the office are just phenomenal, and ALWAYS exhibit such a genuine concern for their patients. If ever an issue arose (from needing to re-schedule an appointment, to a prior authorization), it was always resolved with a sense of urgency. Dr. Reader has the unique ability to be compassionate, fair, stern, and receptive to your individual treatment needs.
This is a delicate balance, that NOT many doctors in this field possess.
This is why I am absolutely grateful to be a patient here, and I thank the Dr., and the amazing staff with ALL my heart!!” – WC

Because from the moment I came into contact with Dr.Reeder and his staff my life got better and I had some hope and self esteem again. I was lucky to discover you and your location is very convenient for me. From the first you’ve been trying to help me and see just what will best help my situation. Dr. R has also encouraged me often with sincere advice, like a brother who really cares. This is SO MUCH better than methadone too, Dr.R’s treatment leaves me lucid and free from withdrawal, not dopey, pale, and high like methadone but normal, focused, and craving-free. This is a reasonably affordable way out of the pain pill chasing game curse. Thanks for treating me like a patient who has suffered from an illness who needs help and understanding. CW
